Provides care to individual or defined populations through the use of telecommunications in accordance with computer based algorithms, protocols, or guidelines. Using the defined process, adheres to nursing standards, company policies and shared principles when providing clinical assessments and health education. Uses the nursing process to identify patient care needs, risk and safety issues, educational opportunities and appropriate health care referrals.Processes calls and conducts in person visits based on quality management guidelines. Demonstrates compliance with all products specific performance metrics. Competency in multiple products/applications may be required.Develops and maintains relationships with providers and provider groups and serves as a liaison between providers, patients and care management teams.May participate as a preceptor and/or resource nurse. Identifies, develops and maintains relationship with the community, health and welfare agencies and state groups that are involved in care of vulnerable populations and are resources for the populations in the program. Provides coordination and assistance to members qualifying for additional benefits available. Provides consultation to the care team for members who are identified as high risk.
Requirements
Qualified candidates must reside in or around the Metro/Boston, MA area. Current MA RN license in good standing. Minimum of 3 years with clinical experience (preferably in a field environment). Formal inpatient or outpatient psychiatric experience. Entry level clinical telephonic and field experience may include basic knowledge of nurse call center procedures and/or company products. Registered Nurse with active, unrestricted Massachusetts RN license. Case Management certification preferred, but will consider strong CM background and eligibility to become CM certified within 2 years.
